Form 1042-S instructions: a filer-side orientation

How the form is organized

The 2026 recipient block places the recipient's U.S. TIN, chapter 3 and chapter 4 status codes, GIIN, foreign tax identification number, LOB code, account number, and date of birth in boxes 13i through 13p (https://www.irs.gov/pub/irs-pdf/f1042s.pdf). The code system

The instructions use income codes for the type of income, chapter 3 and chapter 4 status codes for the withholding agent and recipient, and exemption codes for the reported withholding treatment (https://www.irs.gov/instructions/i1042s). The unique form identifier

A withholding agent assigns a unique form identifier to every original Form 1042-S filed for the year. It must be numeric, exactly 10 digits, unique to that original form for the year, and different from the recipient's U.S. TIN or foreign tax identification number (https://www.irs.gov/instructions/i1042s).

An amended Form 1042-S uses the same unique form identifier as the original. This connects the amendment to the correct return when multiple forms exist for the same recipient (https://www.irs.gov/instructions/i1042s).

Amendments

The current form face includes an AMENDED checkbox and an amendment number field. The instructions require the original form's unique form identifier on the amendment (https://www.irs.gov/pub/irs-pdf/f1042s.pdf and https://www.irs.gov/instructions/i1042s).

Recipient copies also require consistency. A substitute Form 1042-S furnished as Copy B, C, or D must conform in format and size to the official form and contain the same information as the copy filed with the IRS (https://www.irs.gov/pub/irs-pdf/i1042s.pdf). The workflow for a specific amendment, including any related Form 1042 change, belongs with the client's preparer.

Extensions

Form 8809 requests an extension to file Forms 1042-S. The current Form 8809 places Form 1042-S in the automatic 30 day class when the request is filed by the due date. Forms W-2 and 1099-NEC are the criteria-gated exceptions, and Form 1042-S is not listed among those exceptions (https://www.irs.gov/pub/irs-pdf/f8809.pdf).

Form 8809 is not used to extend Form 1042 or to extend the time to furnish statements to recipients. Form 1042 uses Form 7004, which extends the time to file and does not extend the time to pay tax (https://www.irs.gov/pub/irs-pdf/f8809.pdf and https://www.irs.gov/individuals/international-taxpayers/discussion-of-form-1042-form-1042-s-and-form-1042-t). The preparer decides whether and when to request an extension.

Filing cautions from the instructions

The instructions warn that a filer submitting returns electronically should not file the same returns again on paper because duplicate filing may generate penalty notices (https://www.irs.gov/instructions/i1042s).
